What is collected

The contact form collects a name, company name, work email, phone number, an optional employee count, and an optional message. A submission timestamp is recorded with each message.

No health information is collected anywhere on this site. There are no questions about medical conditions, dates of birth, Social Security numbers, or employee census data. Do not send that information through this form.

How it is used

• To reply to the request and arrange a consultation.
• To prepare options and proposals for the company that asked.
• To keep a record that consent to be contacted was given, and when.

Information submitted through this site is not sold, rented, or traded.

Analytics cookies

Who sees the information

Submissions are emailed to Dunn Benefits Group. Information is shared with insurance carriers only where it is needed to quote or place coverage the company has asked for, and with service providers that host this site and deliver its email.

How long it is kept

Enquiries are kept for as long as needed to serve the relationship and to meet record-keeping obligations that apply to insurance producers.
